NHS HEALTH CHECK 

The surgery is offering patients between the age of 40-74 a NHS Health Check. You may receive a letter inviting you for a check. If you would like to be seen please contact reception. The appointment will take around 15 minutes and clinics are run by the Health Care Assistant.

The NHS Health Check is designed to help prevent heart disease, stroke, diabetes and kidney disease.

The appointment will involve basic measurement taking such as height and weight together with diet and lifestyle advice. You will then be given a fasting blood test form to have your cholesterol checked. 

This will enable your doctor to predict if you are at risk of heart disease, stroke, diabetes or kidney disease.

If you suffer with Heart Disease, Diabetes, Kidney Disease, High Blood Pressure, High Cholesterol, Atrial Fibrillation or have suffered a Heart Attack, Stroke or Transient Ischaemic Attack (TIA) in the past then the NHS Health Check is not suitable for you and you will need to see your GP if you would like a review.
